Wellness Plan - Terms of Service
This document sets out the terms of services in relation to the membership of Mount Pleasant Veterinary Group Wellness Plan. Mount Pleasant Veterinary Group is a business unit of Mars Veterinary Health. By completing and submitting your Wellness Plan membership application form ("Membership Application Form") you confirm that: (i) you are the legal owner of the pet(s) listed on your Membership Application Form ("Your Pet"); (ii) you wish to enter into a contract for membership of Wellness Plan with Mount Pleasant Veterinary Group ("us", "our", "we"); and (iii) you agree to these terms of service.
Please read the terms of service carefully. If you have questions concerning them, please raise these with us before submitting your completed Membership Application Form. If you would like a copy of these terms of service in larger print, please ask at our reception or visit our website.
1. Agreement
1.1 These terms of service and the Membership Application Form (together the "Terms") will form the basis of our contract with you.
1.2 In the event of a conflict within the Terms, these terms of service will prevail.
2. Treatment
2.1 When you become a member of Wellness Plan, Your Pet is entitled to receive treatment and other benefits set out in the Membership Application Form.
2.2 Wellness Plan is a preventative healthcare program for Your Pet.
2.3 WELLNESS PLAN IS NOT AN INSURANCE POLICY. IT DOES NOT COVER ALL THE TREATMENT THAT YOUR PET MIGHT REQUIRE.
2.4 As a member of Wellness Plan you are entitled to receive preventive healthcare as specified in the Membership Application Form. Full details of the cost of membership and the services and other benefits you receive by joining Wellness Plan are available in the Terms and on our website at www.mountpleasant.com.sg.
2.5 It is your responsibility to utilize the benefits applicable to your Wellness Plan membership within the 12-month period. You shall not be entitled to any refund for any benefits under the Wellness Plan that you do not use.
2.6 The benefits of your Wellness Plan can only be redeemed at the specific Mount Pleasant branch where the plan was purchased.
2.7 Flea and worm products or services may be collected in accordance with the recommended pack size. The flea and worm products or services may only be given in a recommended pack supply for Your Pet and although we may send you a reminder to collect the recommended pack size it is your responsibility to collect them. We shall not be responsible for any loss caused by your failure to collect them. Any recommended pack sizes may be reviewed and amended from time to time by us in our reasonable discretion, but we shall provide written confirmation to you.
2.8 Vaccination reminders will be sent to the client every 12 months. It is the client's responsibility to ensure the practice has correct contact details. It is the responsibility of the client to ensure that all vaccinations are done at the correct time. In the event of late vaccination, resulting in the full vaccination course having to be restarted, an additional fee will be charged.
2.9 Two Veterinary Consultations applies to two standard / First-opinion vet consultations per year and is for routine veterinary consultations during normal working hours and cannot be used for Referral or Emergency Service consultations. The practice reserves the right for all consultations to be booked with veterinarian of their choosing. ‘Two Veterinary Consultations’ must be used within the current 12-month period and are non-refundable.
2.10 The purchase limit for Royal Canin and Hill's prescription diets at a 20% discounted rate is capped at SGD 1,500 per brand within the 12-month period.
2.11 Should Your Pet have routine care or treatment provided by another practitioner independently of us, Your Pet will not be covered by these Terms of the Wellness Plan. We will not be liable for any routine care or treatment provided to Your Pet by another practitioner independently of us.
2.12 Your Pet can still receive treatment by us outside the scope of Wellness Plan. Any additional healthcare, services or treatments which are not included as part of Wellness Plan will incur an additional charge which is payable separately by you to us and subject to our standard terms of service.
2.13 Any products prescribed by our vets and nurses must be used in accordance with their instructions and must not be used on any pet other than Your Pet. If Your Pet is sensitive or allergic to the products prescribed by us, substitute products may be available, which may result in an additional charge. Please discuss Your Pet’s clinical requirements with us.
2.14 Any product or service that may be required for conditions that come to light during a Wellness Plan consultation are excluded and not complimentary (“Conditions”). These Conditions will be subject to a further fee payable by you to us.
3. Your Responsibilities
3.1 You are responsible for following our vets’ and nurses’ guidance, and for ensuring Your Pet attends the practice regularly for their preventative healthcare checks, and for the timely collection and administration of any prescribed treatment.
3.2 It is your responsibility to let us know that you are a member of Wellness Plan when you arrange treatment or purchase goods.
